This is to confirm the booking for Helena Pearson to perform at the above Event on the following terms:
1) The Total Fee is payable as follows - a non-refundable deposit of £100 is payable on the signing of this Booking Form and the balance is payable not less than 21 days before the Event.
2) All payments should be by cheque or online bank transfer. Cheques are made payable to Helena Pearson and sent to 2 Silverdale Road, Northburn Lea, Cramlington, Northumberland NE23 3LW.
3) In the event that Helena is unable to perform at the Event she will inform the Client as soon as possible, in which case she will be under no liability to the Client other than the refund of that part of the Total Fee which has been paid.
4) In the event of a cancellation by the Client, the Client must notify Helena in writing. A booking is not considered cancelled until Helena has received written notification and she has replied confirming receipt of your cancellation notice. If the cancellation is within 6 months of the Event then 75% of the Total Fee is payable. If the cancellation is within 3 months of the Event then 100% of the Total Fee is payable. All monies that become due, because of the cancellation, must be paid within 7 days.
